Survey on Services for Older Adults in Oak Park and River Forest Townships

In the spirit of our values of Connection, Service, Community, and Caring the Townships of Oak Park and River Forest are seeking to gather input from the community on the services we provide and to measure the need for programs and services to support living in our community at any age.

We are seeking information from older adults, their families and caregivers, community partners, and those who work with older adults.  We feel that gathering input from all of these groups will allow for a holistic view to help us shape our vision for Township programs and services moving forward.   

It is estimated that this survey will take approximately 20 minutes to complete.  Your participation in completing this survey is completely voluntary.  While your comprehensive feedback is appreciated, you may exit this survey at any time.
