Campership Application 2026
Camp Ho Mita Koda Foundation believes that every child who desires a place at camp should be able to attend. With the help of generous donors, we are thrilled to be able to provide full and partial financial aid awards each summer. These are extended to campers with a family need and/or new Type 1 Diabetes diagnosis.
Please read through the campership application and complete every question in its entirety prior to submission. Incomplete applications will not be considered.

One form of documentation is required with most applications, depending on the criteria you are applying under. Please have it ready prior to beginning the application:
Family Income: Most recent federal tax return 1040 (page 1 & 2) available to upload.
Receipt of Medicare/Medicaid: Photo of your current insurance card or other documentation
Newly Diagnosed with T1D or Multiple children with T1D: Hospital discharge paperwork or other medical documentation
Receipt of other government assistance: Photo of EBT or WIC card or other documentation
Medical expenses: Receipts or other documentation showing these costs
