About
  • Supporting the Journey Home is a virtual education program for interprofessional community care teams working in First Nations communities.
  • The program will provide practical tips and strategies on how to incorporate an early palliative care approach from a First Nations' perspective.
Program Commitment
  • 9 weekly 1.5 hour sessions
  • Virtual learning (Zoom with audio and camera)
  • Participate in Circle Reflections
  • Complete weekly exercises
Who is this for?
  • Community care providers who have a caregiving role (e.g. physicians, nurse practitioners, nurses, social/mental wellness workers, patient coordinators, PSWs, CHNs, CHRs, cultural workers, Elders etc.)
  • Must be working within a First Nations community setting or coordinating care with community care providers based in a First Nations community (e.g. external care providers coordinating/delivering care for First Nations people)
  • Must be living and working within the province of Ontario

When
We will be offering two sessions every week on Tuesday 1:00 PM to 2:30 PM EST and Thursday 10:30 AM to 12:00 PM EST from January 14, 2025 to March 20, 2025.
