Safe Nights Network: Your Designated Driver Any Time pilot program.
By completing this survey, you may be eligible for a $10 Uber credit. There are 10 Uber credits available for winnings.

- The City of Madison will not sell your data. However, the City of Madison makes no representations or warranties about the use of your data by Uber or other third parties, which is subject to the terms outside agreements.
- Survey results will be saved in an anonymized database, with personal data removed. The anonymized survey results may be subject to open records requests.
- Your email addresses, phone numbers, and other personally identifiable information will be kept separate and is generally not subject to public records requests, as set forth by state law and the City of Madison’s privacy policy here: https://www.cityofmadison.com/policy/privacy. The Uber voucher system is not linked to the survey results database.
- You may opt out from being contacted through this program at any time.
- After the completion of this survey, The City of Madison does not plan to reach out to you about this program again unless you win a $10 Uber credit.
