HFS ADT Platform Implementation Survey - Ambulatory/Community Providers

The Illinois Department of Healthcare and Family Services (HFS) has awarded a contract to Collective Medical Technologies (CMT) for the State ADT System through a competitive bid process. It is a cloud-based solution that will support ADT notifications for the Medicaid program. All ambulatory/community providers that are enrolled in the IMPACT system to serve Medicaid participants will have the opportunity to subscribe to the ADT platform. This subscription will allow them to receive real-time electronic notifications of admissions, discharges, and transfers for their Medicaid customers who have visited a hospital, emergency room or long term care facility. The result: Improved care coordination, higher quality care, and more successful outcomes.
