The HPC Umbrella Hub, operated by Health Promotion Council of Southeast Pennsylvania, helps to connect people who are pre-diabetic, or are at risk of developing type-2 diabetes, with providers of the National Diabetes Prevention Lifestyle Change Program for virtual or in-person services.
