Division of Family Health Provider Survey 2019

The New York State Department of Health Division of Family Health seeks your input to inform a needs assessment for its programs that serve women, infants, children, and families (the Title V Maternal and Child Health Services Block Grant [MCHSBG] and the Maternal, Infant, and Early Childhood Home Visiting Grant [MIECHV]). Responses will be used to identify what helps NYS’s families be healthy and what needs to be improved.

All questions are optional, but it will help us if you answer as many as you feel comfortable with. You will not be identified in any way in any reports. The survey takes about 15-20 minutes to complete.
