Flu season begins each year in October and lasts until May, peaking between December and February. The Louisiana Department of Health has created the "Fight the Flu" initiative to encourage everyone who is eligible to get a flu shot, to do so. 

Please take 1 minute to complete this brief survey about getting a flu shot. Your participation is voluntary and your answers will not be linked to you. The results of the survey will tell us how likely people with developmental disabilities are to get a flu vaccine this season. If you are completing this survey on behalf of someone with a developmental disability, please respond to the questions from that person's viewpoint.
