Document Delivery Service - User Survey

Welcome to the ILLiad Document Delivery Service (DDS) survey!  This survey consists of 14 questions and will take no more than 3-5 minutes to complete.

We are very interested in your feedback as a user of this service and would like to know which features of our current system you use and enjoy, which you do not, and what other ways we might be able to improve our service to you.
