Parent/Caregiver/Guardian Questions

This survey is designed to help researchers better understand the types of services and supports children with cochlear implants use from the time they are identified with hearing loss and throughout the educational life cycle. The data collected will help researchers better ascertain the knowledge base parents, caregivers, and professionals have regarding the key transitions, guiding processes, documentation, roles and advocacy skills needed to develop and implement IFSPs (Individualized Family Services Plans) and IEPs (Individualized Education Plans) which promote positive outcomes for children with cochlear implants and their families.   

If you are a parent, guardian, or caregiver with multiple children who use cochlear implants, please complete the survey for your oldest child with CIs. There is an option at the end to engage with researchers to provide input for your other children. Thank you for your contributions!

If you are a professional, please complete the survey as it pertains to your work with children who use cochlear implants.
