AEC 2026 Student Consent & Permission Form

CSBA Student Board Member Program and AEC and Trade Show | Dec. 2, 2026 | San Diego, CA

This form is required for each student attending the event.

The Parent/Guardian of any student attending the California School Boards Association (“CSBA”) Student Board Member Program during the Annual Education Conference and Trade Show (“AEC”) in San Diego, CA, on December 2, 2026, must provide a signed Student Consent & Permission Form acknowledging and agreeing to all terms for attendance at the event(s) set forth in the CSBA Annual Education Conference online registration to CSBA prior to attendance.

You have the option to access the Student Consent & Permission form through the provided link, where you can elect to electronically sign it or download a copy. Please ensure that the form is fully completed and returned by November 4, 2026. You may complete the form electronically, or alternatively, scan and email it to training@csba.org or mail signed hard copies to California School Boards Association, Attention: Training, 3251 Beacon Blvd., West Sacramento, CA 95691. If you plan to mail the signed copy, it must be postmarked by November 4, 2026.
