DCYF Data & Evaluation Feedback Survey |
Introduction
Thank you for volunteering to provide feedback on DCYF's data and evaluation tools. Since we are starting off the new FY2018-23 funding cycle with significant changes to our Contract Management System (CMS) and Youth Experience Surveys, we would like to give grantees an opportunity to provide feedback on their experience using these new tools.
Currently, this survey can be used to provide feedback on DCYF's CMS and the Youth Experience Surveys. It will remain open all-year long so that you can provide feedback anytime you want. In the future, you will be able to use this survey to provide feedback on DCYF's evaluation studies once those have started. Please note that this survey should not be used to request CMS troubleshooting assistance. If you need this assistance, please email the DCYF Data & Evaluation Team at cmssupport@dcyf.org or contact your Program Specialist.
Also note that while DCYF cannot guarantee that tools will be altered to accommodate all your feedback, this survey will be used to keep a log of all feedback, which we will then use to inform potential adjustments to the CMS and Youth Surveys for future years of the FY18-23 funding cycle.
