The Office of the D.C. Auditor (ODCA), whose mission is “to support the Council of the District of Columbia by making sound recommendations that improve the effectiveness, efficiency, and accountability of the District government,” is reviewing suicide awareness and prevention policies and practices in District schools.
 
Your responses to this survey are confidential.  You are not required to provide your name or the name of the school where you work. You will remain anonymous unless you would like to be interviewed and choose to enter contact information at the end of the survey. Only ODCA has access to the Survey Monkey account being used to administer the survey.
 
Individual responses will not be shared outside of ODCA; aggregated response data may be included in our public report, which will include analysis and recommendations for improving suicide awareness and prevention in schools.
 
ODCA will not publish any information that references a specific school.
 
Your responses to this survey will help us better understand the nature of your role and duties with regard to students’ mental health. Your responses will inform our research and analysis and help us: (1) make actionable recommendations to the Mayor and the D.C. Council for improving mental health services students receive in school, (2) inform public dialogue on this important subject, and (3) foster positive change.
