Maternity & Neonatal Experience Questionnaire

Your feedback will be used to find out what works and what needs improvement in our local maternity & neonatal services.

Please answer these questions if:

A) You, or your partner, experienced maternity or neonatal services in Central Cheshire (Leighton Hospital or Northwich Infirmary), i.e. Mid Cheshire Hospital Trust (MCHT)

AND

B) neonatal, antenatal, birthing, or postnatal services were used by yourself, or your partner, within the past 3 years.

All identifying personal information will remain confidential and not be shared with anyone else. Central Cheshire Maternity & Neonatal Voices Partnership will use your responses to pick up on trends and recurring themes to identify areas to work in partnership with local maternity & neonatal services and professionals to improve.
