This survey aims to explore attitudes and beliefs surrounding dementia, and compare them across the population. It is designed to be taken by people with dementia, their carers and members of the general public. 

All information you provide is anonymous. Results from this survey will be used by Alzheimer's Australia for research purposes and publication. It is important to think carefully and select the response that best applies to you for each question.
