Physician Survey About Patient's COVID19 Vaccine Attitudes

The Dade-County Medical Association Physician Leadership Academy is interested in understanding the challenges that physicians face when addressing COVID19 vaccine misinformation and vaccine hesitancy among its patients.  It has taken other participants about 5 minutes on average to complete the survey.  Your participation is voluntary, and all answers are anonymous and confidential. Completion of the survey implies consent to participate in the study.
