You probably know someone with a mental health issue. One in four of us has one. And, although Dr. Google can tell you all about symptoms and treatment, there's very little info about how to deal with mental health issues in the real world. 
 
It's time to change that! 
 
I’m Ashley Womble, an award-winning journalist on a mission to change the way people think about mental health. I’m writing the first ever guide for Millennials about how we can live an awesome life while dealing with depression, anxiety, and other mental illnesses. I’m calling it “Real Talk About Mental Health.” But in order for this book to start a conversation, it needs more than my voice. 

That’s why I’m asking you to spend 10-15 minutes taking this survey about your own mental health and how it impacts your health, relationships, and everything else in your life. Most of the survey is multiple choice, but there are times when you can go into more detail if you’d like. And if you're really feeling talkative, let me know and I'll interview you for the book.
 
The aggregated results of this survey will be used as part of the research for my book, but your name and the individual answers and information that you provide won’t. Unless you choose to set-up an “on-the-record” interview with me, your answers will be kept completely anonymous and will not be shared with anyone else. The only uses of your email address should you choose to provide it will either be to contact you in the event that you have won the $50 Amazon gift card (if you submit  your email address for that purpose) or to contact you to to set up an interview (if you submit  your email address for that purpose), and I will not use your email address for any other purpose or share it with any other party.  The winner of the gift certificate will be chosen randomly at the conclusion of the survey process from the pool of submissions who provide a contact email.
