Bromley Sparks is an organisation run by and for people with a learning difficulty or disability.

We are working on a new booklet about disability hate crime for people in Bromley. The booklet will help to make everyone aware of what hate crime is and the harm it causes. It will also help people with disabilities know what to do if they are the victim of hate crime.

We want to hear from people with other disabilities in Bromley. This will help us make the booklet as good as possible.

You can fill in this form yourself or ask someone to help you fill it in. It will take about 5-10 minutes.

You do not have to answer every question if it doesn't apply to you. You do not have to give any personal information. If you do include any personal information it will be kept private.

Please do the survey by 30 September.
