Application Instructions

Purpose
Honors a diabetes care and education specialist who has made a special contribution to the field through dedication, innovation and sensitivity in patient care. The Diabetes Care and Education Specialist will act as an "Ambassador" for ADCES and should be someone who is considered a role model by their peers and colleagues.
Award
Recipient will be recognized at the ADCES Annual Meeting with a plaque, will receive complimentary registration to the Annual Meeting and a $5,000 award.

The recipient may select either the current five virtual/face-to-face educational sessions in which he/she is asked to travel, if not a virtual session, to Coordinating Body or Local Networking Group meetings and present an educational session or select the option to present 1-2 webinars and one session at the Annual Meeting, Virtual or In Person, in the year immediately following his/her selection.


Eligibility
Applicant
/nominee must be, for at least 5 years of continuous membership at the time of application, an active member of ADCES as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award.  Current and past members of the Board of Directors and past recipients are not eligible.

At the time of the application submission, the candidate must be a Certified Diabetes Care of Education Specialist or BC-ADM for at least 5 years.
Criteria
Candidates for the Diabetes Care and Education Specialist of the Year may be nominated by a peer/colleague or by self-nomination. 
Application Process

The nominator/applicant must submit
  • Three letters of support, of not more than 1,000 words, from a professional colleague other than the nominator.  To include:
    • How you know the nominee
    • Why you are supporting the nominee for the Diabetes Care and Education Specialist of the Year Award
    • How the nominee has made a special contribution to the field of diabetes education through dedication, innovation and sensitivity in patient care
  • CV or Resume
Applications and supporting materials must be submitted by March 22, 2021. All applications must be complete to be considered.  Please provide as much detail as possible in your answers. Applications demonstrating thoughtful, detailed responses tend to yield more favorable results. If selected, what would be your topic or platform.
