- Supporting the Journey Home is a virtual education program for interprofessional community care teams working in First Nations communities
- The program will provide practical tips and strategies on how to incorporate an early palliative care approach from a First Nations perspective.
Program Commitment
- 9 weekly 1.5 hour sessions
- Virtual learning (Zoom with audio and camera)

- Complete weekly exercises
Who is this for?
This program is open to health and social care providers involved in supporting First Nations, Inuit, Métis, and Urban Indigenous (FNIMUI) individuals, families, and communities experiencing serious illness across Canada. E.g. Physicians, nurse practitioners, nurses, social/mental wellness workers, patient coordinators, personal support workers, community health representatives, cultural workers, Elders, etc.
When
Sessions will be offered virtually on Wednesdays from 10:30am-12pm ET, and Thursdays from 1pm-2:30pm ET. Participants only need to attend one session per week, and can choose whichever day works best for their schedule. We will be offering two sessions each week for nine weeks starting on Wednesday, September 16, 2026.
