Earlier this year, Health Forward Foundation announced a new plan to move the needle on health equity over the next 10 years. Our purpose statement is: Every day we work to support and build inclusive, powerful, and healthy communities characterized by racial equity and economically just systems. 

The change we seek in our outdated systems will not happen overnight. We know change is possible if we come together as a community and strategically focus our efforts to move toward systems that benefit all of us. 

Since we started this process, we have been gathering information and input from a variety of sources. Last year, we conducted interviews and focus groups with more than 100 individuals who helped shape our new direction and strategies. As we continue to build our initiatives, we want to hear from you. Specifically, we want your input on how our communities could change and what impact we should aim for over the next five to 10 years. 

We invite you to share your thoughts with us through the following survey, which includes questions exploring various aspects of our work. Feel free to respond to as many as you want; all questions are voluntary.
