How to complete:

  • There are 7 awards that can receive nominations. These are detailed on the following page, along with a text box for you to enter your nomination.
  • Please enter your nomination under the relevant category.
  • Please give as much information as you can to show how the individual, team or home meets the criteria using no more than 500 words.
  • You should look at the criteria for each award and use this as a guide for completing the nomination.
  • Please include your name and contact details at the end of the form so that we can inform you if the person you nominated has been successful.

FAQs

Who can nominate?
  • Any health or social care professional with links to the care delivery e.g. social workers, GPs, District Nurses and any other visiting professionals (excluding commissioning & regulating colleagues).
  • Any staff and managers working within the Independent Provider.
  • The person (and their family and friends) whom is receiving care from the provider.

When can I nominate?
The nomination period is open 9th September until 11th October.

When will we know if a nomination has been successful?
Nominations to be presented to the judging panel will be informed before Christmas 2024. Judges shortlisted and winners will be revealed on the night.

When is the Awards Ceremony?
Friday 7th March 2025.

How can we attend the event?
Information about ticket sales will be shared with your organisation in December/January.

Ticket cost to be confirmed but will include a welcome drink, hot food and entertainment following the award ceremony.
