Get Offline, Get Outside Challenge Submission Form

Directions:
Complete all 50 items on the Get Offline, Get Outside Activity List before you turn 19! Once you have finished all 50 activities, fill out this form and receive a commemorative sticker from DEC, a year subscription to DEC's Conservationist Magazine, and a downloadable certificate marking your accomplishment. Kids 12-18 will be eligible for end of year prizes.
Rules:

1. Challenge can be completed by anyone under the age of 19.
2. All finishers will receive a commemorative sticker, a year subscription to DEC's Conservationist Magazine, and a downloadable certificate. Only 12-18 year olds will be eligible for additional end of year prizes.
