Completing this form is required in order to cancel OR substitute teachers already registered but no longer attending Laying the Foundation (LTF) training at Frederick Douglass High School (July 9-12, 2019).
Substitutions:
AdvanceKentucky will formally register the new teacher for LTF.
The substituting teacher will receive email confirmation of his/her registration once the substitution has been processed.
For Cancellations:
Cancellations received forty-five days prior to the first day of training are eligible for 100% credit.
Cancellations received thirty days prior to the first day of training are eligible for 50% credit.
Cancellations made within thirty days of the first day of training will be charged the full registration fee
No-shows will be charged the full registration fee.
