The Township High School District 113 is participating in a statewide study of former high school students.  The individuals selected for this study are students who graduated or left school in 2021-22 and who received special education services while they were in high school. The information gathered will be used to assist our district in improving school activities and student outcomes to support future student achievement and post school goals after graduation related to study, training, work, and independent living.
This survey is conducted as part of the statewide effort that is required by federal law, 20 U.S.C. 1416(a)(3)(B) and has been approved by the Illinois State Board of Education (ISBE).  Though this study is required by law, participation is voluntary and all of your responses to the survey will be kept private along with any identifying student information. The questions on the survey will pertain to your high school experiences, continuing education history and your work history.  Your individual answers will not be shared with anyone. The survey should take approximately (10) ten minutes to complete.
The survey can be completed by the student and/or his or her parent(s).  This survey is not meant to be a reflection of a student’s relationship with any staff member, but a learning tool for the District to improve outcomes for students with disabilities.  The more honestly and completely you can answer the questions, the more helpful the results will be to guide our practices for future generations.
