Mobile Crisis Survey

NAMI Connecticut is soliciting feedback from its members and supporters about their experiences with Connecticut’s current 2-1-1 system and Connecticut’s Mobile Crisis Intervention Services (formerly known as EMPS, or Emergency Mobile Psychiatric Services) and the response to their calls when they or their loved ones are in a mental health/suicide-related crisis.

Please complete the following survey questions as best you can. If you have not called 2-1-1 and/or the DMHAS Local Mental Health Authority (LMHA) Network to access Connecticut’s Mobile Crisis Intervention Services, please stop and do not fill out the survey.
