Return Instructions
To send back expired or expiring controlled medication please complete questions below. Items that should be sent back include: injectable vials, unit dose tablets and liquids within 3 months or less than their expiration date and within 1 month or less for tablet/capsule prepacks. Please include a copy of the CDAR in the package along with any medications you send back and keep the original CDARs for your records.
Once this form is submitted you will receive a prepaid shipping label in your email for a flat rate box, which can be picked up for no cost at the post office. You should receive your label within no more than ten days after completing this form. Once you receive your label in your email, please print it and tape it to the flat rate box after packaging up the expiring medications. Once the package is sealed up and the label is on the package, place the package in the mail. The pharmacy will send out replacements for you expiring medication once we receive your package. If you have any questions or you do not receive your label after ten days please email narcoticssecurityofficer@ykhc.org.
