Are you a person, or a carer of a person with Dementia?

The Victorian Department of Health & Human Services has funded the Southern Mallee Primary Care Partnership (SMPCP) to undertake a project to review and enhance the accessibility of health and service information for people who have been diagnosed with dementia and carers of people with dementia in the Swan Hill, Buloke and Gannawarra Shires.

Dementia is not a specific disease, but an overall term that describes a wide range of symptoms associated with a decline in memory or other thinking skills. These symptoms can be severe enough to reduce a person's ability to perform everyday activities.

Gaining a greater understanding of the communities needs in relation to dementia services and supports across the Southern Mallee is a key aim of the project. We invite and encourage people with dementia and their carers from across Swan Hill, Buloke and Gannawarra Shires to participate in the following dementia questionnaire.

The information obtained from the questionnaire will be used to identify and document the gaps, barriers and what is working well in dementia services and to inform future work.

Your privacy is of the utmost importance to us, therefore it is not a requirement you include your name, address and contact details in the questionnaire, unless you choose to. However, we do ask if you could indicate the local government area and town you live in to assist us in the mapping component of the project.
