This survey is for agencies who actively provide DD waiver funded RESIDENTIAL SERVICES. If your agency does not provide residential services, please do not respond. If your agency provides both residential and day array services, please only respond regarding your residential DSPs. Lastly if your agency provides services in multiple counties, please respond regarding ONLY staff in Hamilton County providing waiver-funded residential services to individuals with DD.
