BCHepatitis Network - Community and Hep C Elimination Survey

Two key reference documents for this survey are the Blueprint to Inform hepatitis C Elimination Efforts in Canada and the Blueprint target table, both available on the community website http://community.bchep.org.

BCHN's BC Community - Hep C Elimination website  http://community.bchep.org will host digital resources that result from the survey and anything else related to the community and hep C elimination once they're developed. 

The main purpose of the survey is to gather data to inform the development of digital and print resources highlighting the role of the community in hepatitis C elimination.  Resources will be publicly available digitally and for download.

What the survey asks
This survey asks about your organization's role in hep C elimination in your community (however you define that). The survey asks about the services you offer now and also what hep C-focused services you would provide, given additional resources.  For this reason, a person in management in the organization (or their delegate) is best suited to answer the survey questions.

Your answers are completely anonymous and providing your organization’s name is not required. At the end of the survey are two web links for the opportunity to:

1.     Be further involved in this survey project, e.g. interviews, survey findings updates and opportunities for input, preparation for the HCV Summit.

2.    To put your name in for an appreciation draw for a gift card.
