Dear community member,

We are conducting a survey to better understand the health care needs in our community.

This survey is part of Community Health Needs Assessment (CHNA) conducted by Roper St. Francis, MUSC Health, and Trident United Way in conjunction with a variety of community organizations. Data from this survey will help health care providers and community organizations improve the overall health and wellness of our community. We will help identify community needs, establish priorities for programs and services, and inform strategies to address gaps between critical needs and services.

To encourage broad community participation, the CHNA survey will be available online, sent by mail, and/or distributed at community meetings and local events. In addition, the Community Health Needs Assessment will include other opportunities for input through meetings, focus groups, and interviews with community members and organizations, state and local leaders, major employers, and community health organizations.

This survey is anonymous. We will not know who has completed a survey and who has not. Information collected will not be attributed to any specific person. Your answers will help us understand the top health needs and how we can better serve the community. The survey will take approximately 5 to 7 minutes, but has additional questions for healthcare and social service workers and community resource providers.

A CHNA summary report will be shared with the public once results are analyzed and reviewed.
