* 1. Dear College Student,

Please take a few moments to read and consider participation in a short survey described below.

Nature and Purpose of Research
The purpose of this project is to learn how college students that have been diagnosed with multiple sclerosis navigate the college lifestyle. We are interested in learning about what successes and challenges students with MS have, and what methods students use to face these experiences. By collecting information about the success of these methods, we hope to gain an understanding about what aspects of college life are the most challenging for students with MS, and identify potential methods that are the most effective.

Procedures to be Followed
If I choose to participate in this study, I understand that all I need to do is complete the survey on line.

Time and Duration of Participation
I understand this is a one-time on line survey that will take approximately 10-15 minutes to complete.

Foreseeable Discomforts or Risks
I understand the risks and discomforts are minimal, including the possible inconvenience of taking time to complete this survey and discomfort about discussing my health. If you become distressed due to the content of this survey, please be aware you may end this survey at any time or skip any questions.

Foreseeable Benefits
I understand there are no immediate benefits to completing this survey. However, I do understand the information from this survey will be compiled and used to increase the supports and resources for other individuals with MS.

Confidentiality
All information from the survey is confidential. No identifying information is connected to the data. The ability to collect IP address information has been disabled on the Survey Monkey system.

Right to Withdraw
I understand that my participation in this research is completely voluntary, and I will not complete the survey if I choose not to participate. I also have the right to skip any questions I am uncomfortable answering.

-For questions regarding the rights of human subjects in research, you may contact the Institutional Review Board, (989) 774-6401. No funds have been set aside for medical treatment in the case of injury related to research; however, by participating in this survey, you are not waiving your rights to seek compensation in event of injury or negligence. If you are not satisfied with the manner in which this study is being conducted, you may report (anonymously if you so choose) any complaints to the Institutional Review Board by calling 989-774-6401, or addressing a letter to the Institutional Review Board, 104 Foust Hall Central Michigan University, Mt.
