The City of Chicago is in the process of developing a new Consolidated Plan that will cover the 2020-2024 period for the use of Federal grant dollars, including funds from the following programs:
  • Community Development Block Grant (CDBG),
  • Emergency Shelter Grant (ESG),
  • HOME grant program, and
  • Housing Opportunities for People with AIDS (HOPWA).
One of the components of the Plan is to identify the City of Chicago’s needs and priorities. This survey offers residents an opportunity to express their opinion on what are the most critical community needs and what types of City-managed services should be prioritized over the next five years. If you have more comments that are not captured by the survey - for example, additional unmet needs/ideas of other services/barriers to accessing certain services - you will have a chance to identify those as well. This survey should take approximately 5-10 minutes.

Even if you do not answer every question, please make sure you go to the end of the survey and click "Done" to ensure that your answers are recorded.

If you have any questions about the information in this survey, please contact our office at OBMGMU@cityofchicago.org.

We will be accepting Survey Responses through September 30, 2019.
