Consumer Feedback Survey 2011

Thank you for participating in the 2011 consumer feedback survey for the Ryan White Part A Planning Council (RWPC). This survey should take about 10-15 minutes to complete. This feedback is to understand the needs of the HIV positive clients who use HIV care and prevention services in the Philadelphia area. If you have any questions or concerns about this survey or would like to participate in other activities conducted by the RWPC, please contact us at (215) 574-6760.

This survey is ONLY for HIV positive individuals who use HIV care and prevention services in the Philadelphia eligible metropolitan area. If you know of anyone who would qualify to take this survey, please refer them to the Office of HIV Planning's website (www.hivphilly.org).
