We constantly aim to provide excellent care for our clients.  With this in mind, Four Villages Community Health Centre, Regeneration Community Services and Storefront Humber are exploring a merger. As we look into the opportunities of joining our organizations, we want to hear from you and ask for your ideas on how we can shape a future merged organization that helps you get the care you want.  

We also want to hear about any concerns you have so we can address and answer them as we move forward with exploring a proposed/potential merger.

Four Villages Community Health Centre provides primary health care and health promotion programs and services (like doctors, nurses, therapists, social workers, physiotherapists, dieticians)

Regeneration Community Services provides support (case management and peer support) and supportive housing services for persons living with mental health and addictions

Storefront Humber offers a wide range of community supports and home care services to seniors and adults living with disabilities

We all provide services in the same neighbourhoods and communities in West Toronto, and by merging we could:

•       Offer a larger team of staff providers and services with easier referral processes and seamless transfers between providers

•       Provide direct access to more services with one phone number to call for you to make appointments

•       Coordinate your care and services if you need it e.g. your providers would have your information at all your appointments so you don’t need to repeat your story many times
