City of Port Orange ADA Transition Plan Public Access Survey

UNDER FLORIDA LAW, THIS SURVEY AND ALL RESPONSES PROVIDED BY YOU ARE PUBLIC RECORDS AND MAY BE RELEASED IN RESPONSE TO A PUBLIC RECORDS REQUEST.  IF YOU DO NOT WANT THE INFORMATION YOU PROVIDE TO BE RELEASED TO THE PUBLIC IN RESPONSE TO A PUBLIC RECORDS REQUEST, PLEASE DO NOT PROVIDE IT.
This survey is designed to help the City of Port Orange locate areas of most significant concern to you, our public, and help us provide better access throughout our community.

7.Have you, a family member or a loved one encountered locations where curb ramps are missing or inaccessible?
8.Have you, a family member or a loved one encountered inaccessible sections or poor conditions related to Votran transit stops within the City of Port Orange?
9.Do you, a family member or a loved one have difficulties accessing public schools within Port Orange due to inaccessibility of sidewalks or curb ramps in front of the school?
10.Have you, a family member or a loved one encountered street or intersection crossings near a City building or park where lack of pedestrian crossing signals or medians affect your ability to cross the street?
