Informed Consent for Adult Participants

You are invited to participate in the research project titled “Relationships Between Interoceptive Body Awareness, Health Anxiety, and Identity and their Predictions of Psychological Factors Associated with Chronic Illness” conducted by Shane George under the supervision of Dr. Ross Krawczyk of the Psychology Department at the College of Saint Rose. You must be at least 18 years old to participate.

The purpose of this study is to further understand the psychological aspect of living with a chronic illness. Upon accessing the Survey Monkey link, you may be asked to describe the symptoms or activities associated with your particular chronic illness. Additionally, you will be asked to complete a demographics sheet on Survey Monkey, and questionnaires about interoceptive awareness, illness attitudes, illness identity, stigma, depression, and self-disgust.

Since recalling details about your physical symptoms can be particularly emotionally arousing, participation is completely voluntary and you can withdraw from the study at any point. You are not required to answer any question that you do not wish to answer. Additionally, your responses will remain confidential. At the end of the questionnaire, you will be asked to indicate your name and email. This is completely voluntary, but doing so will enter your name into a raffle to possibly receive one of twelve $100 Amazon gift cards if selected after data collection for the study has ended. Your personal information will never be associated with your responses; this will be used to contact you if your name is selected as one of the raffle winners.
