Seattle Children's Hospital COVID-19 PPE access

Nurses deserve the appropriate PPE to safely care for our patients in this pandemic.

There is an active investigation by Division of Occupational Safety and Health (DOSH) of Labor and Industries (L&I) based on a safety complaint filed by WSNA regarding concerns of nurses’ access to appropriate PPE when caring for suspected and positive COVID-19 patients.

Please fill out this form to document your concerns for WSNA to submit to the DOSH investigator. It is important

2.Have you been providing direct care to diagnosed COVID 19 patient(s)?
3.Did you have access to N95, PAPR or CAPR when performing aerosolized procedures for these patients?
4.Did you have to use the same N95 for more than one patient?
5.Did you have access to a mask for all patients in droplet precautions?
6.Did you have to use same surgical mask for an entire shift?
7.Have you been providing care to suspected COVID 19 patient(s)?
8.While caring for a suspected COVID 19 patient did you have access to the any of the following:

9.Have you requested to be tested for COVID 19?
10.If you were exposed to COVID-19 patients, were you notified by your employer to the exposure?
11.Have been tested for COVID 19?

13.Have you been given verbal instructions from a manager that contradicts hospital policy on PPE?
14.I would like to participate in the DOSH investigation interviews.
