Never - a patient should not be charged for communicating with a physician via a physician or entity-provided portal

A patient should be charged if an insurer doesn't pay for electronic communications via the portal

A patient should be charged if the communication is not related directly to their medical care including current episode of care

A patient should be charged if the question is related to an administrative matter (completing forms, providing copies of medical records, etc)
