Please complete this online form only if you have successfully completed and received final grades for ALL classes required for your certificate. If this is the case, you can expect to receive your certificate via secure email through a conferral platform called Accredible.

Certificates are conferred electronically at the end of each month. Please allow up to one month from your request to receive the conferral information, and be sure to check your spam box if the conferral notification is not available in your inbox.
