COVID-19 Pre-Screening Survey

To prevent the spread of COVID-19 and to help protect others, it is requested that each member complete this self assessment survey every week before attending worship service at the building. The survey is designed to help assist individuals with determining their risk of having COVID-19 or exposing others. It is not designed for the diagnosis or treatment of COVID-19 or any other condition.

The self assessment survey must be completed on the day you are planning to worship at the building. It is expected that all questions are answered honestly.

The personal information collected is only used by Central Church of Christ Health Ministry for the purposes of minimizing COVID-19 exposure. No information is shared with any other parties.
