Cempa Community Care has launched a Community Health Needs Assessment (CHNA) to better understand our community's health and identify factors that may affect your wellbeing. The CHNA looks at the overall health of Hamilton County and the availability of resources to address community health needs. As part of this effort, we are conducting a community health survey and greatly value your thoughts. Please take a few minutes to complete this anonymous survey, which will be analyzed by StrategyHealth, our Community Health Needs Assessment partner. Please complete by June 1, 2026.
