The Champlain Hospice Palliative Care Program (www.champlainpalliative.ca) invites you to complete this short application to attend our next 5-week virtual Practical Caregiver Training.

The next training will run on Tuesday evenings from January 12 - February 9, 2021 from 6:30 - 8:30 p.m.

After you complete this application, you will be contacted via email to confirm your spot and provide you with more information about the program. 

Your information will be kept private and confidential and will only be used for the purposes of this program. It will never be shared with any other organization.

January 12. 2021: Your Role as a Caregiver
January 19, 2021: Lifts & Transfers <--- Special Guest Physiotherapist!
January 26, 2021: Providing Personal Care <--- Special Guest Nurse!
February 2, 2021: End of Life Care  <---Special Guest Nurse!
February 9, 2021: Communication & Self Care
What you will need for this course:
1. A stable internet connection
2. A computer/tablet/phone that is connected to the internet
3. A private/quiet area for you to connect to the training
4. Strongly recommended: Earphones with a Microphone (this is encouraged for confidentiality and privacy for yourself and all the other participants).
