This is a repeat survey to continue to understand the community’s perception on cannabis and pregnancy. Everyone 16 and older who lives within Thunder Bay and District is welcome to complete this survey and enter in the draw at the end of the survey. We appreciate you taking the time to provide your thoughts around cannabis, as well as cannabis use during pregnancy. This survey takes about 5-10 minutes to complete and has about 20-30 questions. This survey will be open until Monday, March 16, 2020 @ 11:59 pm EST.

Your participation is voluntary. By completing this survey you are agreeing to have your answers used by the Thunder Bay District Health Unit to research and report on community perceptions about cannabis in general as well as cannabis use during pregnancy.

All information will be held in the strictest confidence. No personal names or identifying information will be used in any analysis, publication or presentation. Once you have completed the survey, you will be directed to a separate page where you may provide your contact information to enter the draw for one of two $50 gift cards. For the purposes of this survey, Cannabis refers to marijuana and includes all forms, such as smoking, edibles, or oils, whether used for recreation or medicinal purposes.
