CYO Hall of Fame & Recognition Award Nomination Form

We are currently seeking nominations for individuals to receive the CYO Recognition Award and special individuals to be elected to the CYO “Hall of Fame”. Nominations for both awards must be submitted to the CYO Athletic Office, 7911 Detroit Avenue, Cleveland, Ohio 44102.

Nominations are accepted into Late September.

We are also asking you to help us publicize the opportunity to nominate by including the news in your church bulletins, newspapers, website or other locations.

RECOGNITION AWARD - Nominee must have provided excellent service to the CYO athletic program and its participants as a coach, parish athletic director, clergy, league commissioner, site supervisor, or any other member of our community whose accomplishments deserve to be recognized.

HALL OF FAME - Nominee must have made significant contributions to the CYO athletic programs as a player, coach, parish athletic director, clergy, league commissioner, or other member of our community whose accomplishments have been of the highest caliber over an extended period of time.
