Cohort 6 CYBHI Fee Schedule Program Readiness Application (10/2025)

Welcome

Welcome to the CYBHI Fee Schedule program readiness application. To apply, entities must be certified public educational institutions, including either:

Local educational agencies (LEAs), including county offices of education (COEs), school districts, charter schools, California Schools for the Deaf, and the California School for the Blind;
or Public institutions of higher education (IHEs), including California Community Colleges, California State Universities, and University of California campuses.

LEAs and IHEs in California that seek to join this Cohort of the CYBHI Fee Schedule program must complete the CYBHI Fee Schedule Program Readiness Application (this application) by 5:00 PM on January 16, 2026.

By completing this application, LEAs and IHEs are applying to join this Cohort of the CYBHI Fee Schedule program and will be able to submit claims with a date of service starting July 1, 2026, pending completion of onboarding requirements.

Note: This application is non-binding. DHCS will work with LEAs and IHEs to successfully onboard onto the CYBHI Fee Schedule program.
