The California Youth Advocacy Network (CYAN) is a statewide training and technical assistance organization which provides support to those working with youth and young adults on tobacco-related initiatives.  The purpose of this assessment is to collect your thoughts on CYAN’s youth, college, and military programs; identify needs for new or revised resources, trainings, and technical assistance; and evaluate local engagement of youth and young adults. Even if you have not worked with CYAN in the past, your thoughts and opinions are valued and we would appreciate receiving your feedback.

The assessment will take approximately 5 minutes to complete. We do ask you to identify the organization or school you represent; however, this information is being collected for internal purposes only and your response will remain anonymous in all reports and presentations where these data may be used.  At the conclusion of this survey, you may choose to indicate your name, organization, and email to receive more information about CYAN’s programs and activities and to be entered into a drawing to receive a gift card.

If you have any questions about the assessment, please don’t hesitate in contacting Kim Homer Vagadori, CYAN Project Director, at kim@cyanonline.org or (916) 339-3424 x22.

Thank you for completing this assessment!
