I understand and consent to the following: That should I be eligible for participation in this market research focus group or interview, my contact information (phone, email, and home address) may be shared with the client for purposes related only to this particular project and are not limited to: screening calls to further determine eligibility, sending of "homework" or pre-session assignments via email, allowing access to an online platform via email, or for in-home interviews conducted at your residence.

* 2. Thanks again for your interest in our study on Entertainment with Males & Females ages 25 to 54.

The groups will be held online on Tuesday, October 22nd and Wednesday, October 23rd for 90 minutes.

You MUST participate using a computer with a webcam (no smartphones or tablets).

You will be paid $160 for your time.

Please check off which times, if any, you'd be available to participate. Each group has different qualifications so make sure you check off all times you'd be available.

All times are listed in Pacific Standard Time.
