Parent Survey
This survey is for all parents of our House League and DS programs and serves as an important guide to our goal of making the Oakville Hornets Girls hockey experience better for our players. We will use the results to help us select the right people to coach, the right initiatives to develop and the right programs to continue.
All parent surveys will be tabulated confidentially, and reported on a consolidated basis. The identity of all respondents will remain confidential.
Please include your name, email and phone number so that concerns might be discussed, and the Director of House League will be able to contact you if necessary.
