Please review the CAPCE Program information, including dates, components and selection criteria at www.palliativecareconsultation.ca prior to completing this application. All components must be completed in order to qualify for your certificate.
 
Mandatory Components of CAPCE include:
• Completion of Fundamentals Enhanced
• Self-directed reading and reflection
• 6 e-learning modules
• 4 in-person, case-based learning sessions (4 hours in length)
• 5 in-person, individual or small group coaching sessions (two hours in length, dates and times TBA)
• 4 peer-to-peer exchanges (learning debrief)
• 2 practical application assignments
Approximately 5 hours of independent study time per week

This course will take approximately 5-6 months to complete.  If the learner is not meeting the mandatory components for the course a contract commitment may be required.

Fundamentals of Hospice Palliative Care is a pre-requisite for CAPCE (some exceptions may be considered on an individual basis). This application form must be completed by the nurse applying to take CAPCE. It will take approximately 30 minutes to complete and must be done in one sitting. It cannot be saved and retrieved at a later time.

If you have not completed a required question, you will be directed back to the missed question.  All applications will be reviewed by the CAPCE team. Due to the large volume of applicants, acceptance into the course is based on your answers to the questions asked in survey monkey.   Those who are accepted will receive notification by email with a link for payment by Jan. 6, 2020. Deadline for payment is Jan. 20, 2020
